Job Summary:

The Care Coordinator supports members navigating the healthcare system, ensuring they receive appropriate care in the right setting. This role involves guiding members and their families to meet with the right specialists, inside or outside the center, in a timely manner. A suitable candidate will have a strong understanding of the healthcare ecosystem, excellent attention to detail, and a passion for providing quality care to seniors.

Duties/Responsibilities:
  1. Partner with the care team to understand each member's needs, identifying gaps and barriers to treatment.
  2. Coordinate referrals to external providers, vendors, and services to meet care needs.
  3. Ensure accurate and current documentation for member referrals.
  4. Build relationships with members and their families to facilitate care coordination.
  5. Monitor activities and progress according to the care plan, ensuring follow-up and goal achievement.
  6. Adhere to HIPAA standards and confidentiality of PHI.
  7. Promote communication among care team and clinicians for positive outcomes.
  8. Assist members with problem-solving related to healthcare, financial, or social barriers.
  9. Serve as the system navigator and point of contact for members and families.
  10. Perform additional duties as assigned.
Required Skills/Abilities:
  1. Deep understanding of the healthcare ecosystem, including benefits and insurance authorization.
  2. Passion for helping seniors successfully carry out care plans.
  3. Effective collaboration with diverse populations including staff, providers, and members.
  4. Basic medical terminology knowledge.
  5. Understanding of Medicare criteria for equipment and supplies.
  6. Knowledge of CPT and ICD-10 codes.
  7. Familiarity with specialists and community resources.
  8. Strong communication and rapport-building skills.
  9. Ability to multitask and manage priorities with high attention to detail.
  10. Proficiency in Microsoft Office.
  11. Fluency in Spanish or languages spoken by the community (if necessary).
Education and Experience:
  1. High school diploma or equivalent; healthcare coursework preferred.
  2. At least 2 years of experience in referral management or care coordination.
  3. Experience as Medical Assistant or in medical administration is helpful.
  4. Experience with EMR systems, preferably eClinicalWorks or similar.
