Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved. Interacts with members by performing outreach via phone or home visits and documents the care/services plan and activities.
Requires a High School diploma or GED.
Requires 1 – 2 years of related experience.
$17.50 - $27.50 per hour.

Actual pay will be adjusted based on skills, experience, education, and other factors. Total compensation may include additional incentives.
