Care Initiatives in Dubuque, IA is seeking an Admissions Coordinator to drive census growth and manage the admissions pipeline end-to-end.

You will build relationships with hospitals and referral sources, coordinate assessments, and ensure compliant, timely placements while delivering a positive intake experience for residents and families. This full-time role blends outside sales, account management, and admissions operations with strong CRM usage and teamwork with the DON and business office.

Qualifications

• Requires experience with referral management systems or CRM.
• Proficiency with Microsoft 365 (Outlook, Excel, Teams).
• Two or more years in post-acute admissions, healthcare sales, case management, or discharge planning.

Responsibilities

• Develop weekly market coverage with hospitals, SNFs, and care managers.
• Maintain account plans and identify decision makers for priority sources.
• Present clinical capabilities and bed availability; conduct in-service education.
• Manage the CRM/referral log and report pipeline metrics weekly.
• Coordinate insurance verification and authorizations for Medicare Advantage and commercial plans.
• Analyze trends and adjust outreach to hit targets.
• Coordinate admissions, assessments, and room readiness.
