Qualifications

• Two+ years in post-acute admissions, healthcare sales, case management, or discharge planning.
• Proficiency with CRM or referral management systems.
• Bachelor’s or associate degree preferred.
• Healthcare experience helpful but not required.

Responsibilities

• Develop and execute weekly market coverage to hospitals, SNFs, assisted living, home health, hospice and physician practices.
• Maintain account plans; identify decision makers and barriers; set SMART goals per source.
• Present clinical capabilities, bed availability, and admit criteria; conduct in-service education.
• Manage CRM/referral log; log activities, track stages and report metrics weekly.
• Coordinate with central intake to verify insurance and accelerate authorizations.
• Analyze trends and adjust outreach to hit targets monthly and quarterly.
