Job summary

AMIkids Inc. is seeking a Case Manager to oversee service planning and transition processes for youth entering and exiting the program. You will tailor individualized plans and monitor progress to ensure effectiveness, empowering youth to achieve their potential.

The role involves intake assessments, developing care plans, participating in MDTs, applying evidence-based treatment models, and representing the program at court hearings and youth proceedings.

Qualifications

• Bachelor’s degree in psychology or social work or related field.
• Minimum 2 years’ experience working with youth.
• Experience with evidence-based treatment approaches preferred.

Responsibilities

• Complete intake needs assessments, re-assessments, and transition planning.
• Develop and document an individualized service plan for youth.
• Participate actively in the Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment models with fidelity.
• Represent the program at court hearings and youth proceedings.
