Job summary

AMIkids in Florida is seeking a Case Manager to oversee intake, service planning, and transitional processes for youth entering and exiting the program. You will develop individualized plans and monitor progress to ensure effectiveness.

You will participate in MDTs, document treatment plans, deliver evidence-based care, and attend court hearings as needed to support youth outcomes. A 2+ year youth-focused background is preferred.

Qualifications

• Intake and reassessment processes for youth.
• Ability to develop and document service plans.
• Participation in Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment with fidelity to models.
• Attend court hearings or youth proceedings when required.

Responsibilities

• Complete intake needs assessment, re-assessments, and transition planning.
• Evaluate, develop, and document an individual service plan.
• Active participant in MDT meetings and planning.
• Deliver evidence-based treatment with fidelity to models.
• Attend and represent the program at court hearings and youth proceedings.
