Job summary

AMIkids is seeking a dedicated Case Manager to oversee service planning and transitional processes for youth entering and exiting our programs. You will create individualized plans, monitor progress, and collaborate with a multidisciplinary team to ensure effectiveness and compliance with best practices.

The role requires a Bachelor’s degree in psychology or social work and at least two years of experience working with youth.

• Minimum 2 years’ experience working with youth.
• Excellent communication and organizational skills.

Responsibilities

• Complete intake needs assessments, re-assessments, and transition planning.
• Evaluate and document individualized service plans based on needs assessments.
• Active participant in the Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment models with fidelity to protocols.
• Attend and represent the program at court hearings and youth proceedings.

We are seeking a Case Manager to oversee the service planning and transitional process for youth entering and exiting the program. What you will be doing:

• As a Case Manager, you will be complete intake needs assessment, re-assessments, and transition planning,
• Evaluate, develop, and document an individual service/care plan for the youth based on the needs assessment, and complete required updates.
• An active participant in the Multidisciplinary Treatment Team (MDT).
• Deliver Evidence Base Treatment models adhere to fidelity requirements of evidence-based treatment model fidelity and documentation.
• Attend and represent the program at court hearings, meetings, and youths’ legal proceedings.

Role requirements:

• Bachelor’s degree in psychology, Social work or related field,
• Minimum 2 years’ experience working with youth.
