Job summary

Project Hospitality in New York, NY is seeking a Case Manager for weekend overnight shifts (Sat-Sun-Mon 11pm–7am) in its Outreach and Housing Placement Program. You will outreach to homeless individuals, assess needs, and coordinate referrals and placements.

Your duties include documenting psychosocial histories, assisting with entitlements forms, and driving the outreach van to ESU and detox as needed. Bilingual Spanish/English preferred; a valid driver's license is required.

Qualifications

• HSD/GED required with 1–2 years experience with the homeless population.
• Bachelor's Degree preferred.
• Experience with the homeless, substance abuse, and mentally ill populations is needed.
• Excellent client engagement skills are a must.
• Must possess a clean and valid driver's license and the ability to drive a passenger van.
• Bilingual Spanish/English preferred.

Responsibilities

• Connect with homeless people living on Staten Island.
• Make assessments and referrals including psych evaluations, TB and HIV testing, detox, etc.
• Assist clients in filling out forms for entitlements, etc.
• Complete psychosocial and social history when applicable.
• Counsel clients leading to interim, transitional, and permanent placements.
• Handle all paperwork and complete accurate timely statistics as required by the program.
• Respond to calls from the community regarding homeless individuals in all areas of Staten Island.
• Drive and escort clients to the ESU and detox as needed using the Outreach van.
• Participate in weekly supervision, case conferences, and staff meetings.

Case Manager – Weekend Overnight - OutreachWork Schedule: Saturday, Sunday, Monday 11pm to 7amThe Case Manager will work within our Outreach and Housing Placement Program, providing outreach to homeless people living in or near the North Shore area of Staten Island and responding to community calls regarding outreach to other areas of Staten Island.Responsibilities:Connect with homeless people living on Staten Island.
