Job summary

Project Hospitality in New York seeks a Case Manager for the Outreach and Housing Placement Program. You will conduct outreach to homeless individuals on Staten Island, perform assessments, and coordinate referrals for services including psych evaluations, TB/HIV testing, detox, and placement options.

Responsibilities include driving clients with the Outreach van, completing required forms and histories, and contributing to supervision and team meetings.

Qualifications

• 1–2 years experience with the homeless population.
• Experience with homeless, substance abuse, and mentally ill populations.
• Clean and valid driver's license and ability to drive a passenger van.
• Bilingual Spanish/English preferred.
• Excellent communication skills both oral and written.

Responsibilities

• Connect with homeless people on Staten Island and assess needs.
• Provide referrals including psych evaluations, TB/HIV testing, detox, etc.
• Assist with entitlements forms and psychosocial histories.
• Coordinate interim, transitional, and permanent placements.
• Complete paperwork and timely statistics for the program.
• Respond to community calls about homeless individuals in SI.
• Drive and escort clients to ESU and detox as needed using the Outreach van.
• Participate in weekly supervision, case conferences, and staff meetings.
