Job summary

Project Hospitality is seeking a Case Manager for the Outreach Housing Placement Program in Staten Island, NY. The role involves direct outreach to homeless individuals, assessments, referrals, and facilitating placements with a focus on coordinated community services.

Responsibilities include facilitating entitlements, completing psychosocial histories, and driving clients as needed. Strong communication, bilingual Spanish/English, and a valid driver’s license are preferred for this role.

Qualifications

• BA/BS or HSD/GED and 2 years of case management experience.
• Experience with the homeless, substance abuse, and mentally ill populations.
• Excellent outreach and client engagement skills.
• Clean and valid driver’s license and ability to drive a passenger van.
• Bilingual Spanish/English preferred.
• Excellent communication skills both orally and in writing.

Responsibilities

• Connect with homeless individuals on Staten Island; make assessments and referrals including psychosocial evaluations.
• Assist clients with entitlements and complete psychosocial histories.
• Counsel clients toward interim, transitional, and permanent placements.
• Handle paperwork and timely statistics as required by the program.
• Respond promptly to community calls about homeless individuals in Staten Island.
• Drive/escort clients to ESU and detox using the Outreach van.
• Participate in weekly supervision, case conferences, and staff meetings.
