Job summary

Project Hospitality is seeking a Case Manager for the Outreach and Housing Placement Program in the Staten Island area. You will engage homeless individuals, assess needs, arrange referrals, and support transitions to interim, transitional, and permanent housing placements.

The role requires HSD/GED with 1-2 years of homeless services experience, with a preference for a Bachelor's degree. A valid driver's license and strong bilingual English/Spanish communication are essential for effective

Qualifications

• HSD/GED required with 1-2 years experience with the homeless population.
• Bachelor's Degree preferred.
• Experience with homeless, substance abuse, and mentally ill populations is needed.
• Excellent outreach and client engagement skills are a must.
• Must possess a clean and valid driver's license and the ability to drive a passenger van.
• Bilingual Spanish/English preferred.
• Excellent communication skills both orally and in writing with individuals and groups on sensitive issues.

Responsibilities

• Connect with homeless people living on Staten Island.
• Make assessments and referrals including psych evaluations and TB/HIV testing.
• Complete psychosocial and social history when applicable.
• Counsel clients leading to interim, transitional, and permanent placements.
• Handle paperwork and statistics for the program.
• Respond to community calls regarding outreach to Staten Island.
• Drive and escort clients to ESU and detox as needed with Outreach van.
• Participate in supervision, case conferences, and staff meetings.
