Job summary

GRO Community in Chicago is seeking a dedicated Case Manager to support individuals through intake and program participation. You will ensure clients meet admission criteria, receive services, and are assisted in overcoming housing instability and legal barriers while coordinating with partners.

The role requires strong advocacy, requires collaboration with probation officers, clinicians, and housing specialists, and involves wraparound support to promote client well-being.

Qualifications

• Bachelor’s degree in relevant field; Master’s preferred.
• 1–2 years in case management/social services or related work.
• Trauma-informed care and harm reduction knowledge.
• Able to work independently and in a team.

Responsibilities

• Conduct intake assessments and create individualized service plans.
• Provide case management and advocacy throughout engagement, including post-program follow-up.
• Connect clients with housing, employment, legal, food, mental health resources.
• Meet participants monthly to review progress and provide guidance.
• Provide educational materials, safety planning, and referrals for independence.
