Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or related field required.
• IM+CANS Certification required at hire or within one month.
• Minimum 2 years’ experience in a similar role.

Responsibilities

• Conduct structured clinical interviews and complete standardized assessments to determine needs, eligibility, and level of care.
• Collect psychosocial, behavioral, and functional information to support care recommendations.
• Accurately document findings and submit assessments in a timely manner.
• Monitor weekly productivity and submit performance data.
