Job summary

CAMBA is seeking a Case Manager in the Bronx, NY 10452 to maintain professional relationships with clients, perform initial intake, assess needs, and coordinate services. The role requires HIPAA compliance and experience with the SPMI population, with flexible hours and a fingerprint-based background check.

Responsibilities include developing independent living plans, advocating for tenants, arranging referrals, and maintaining thorough documentation for reporting and program continuity.

Qualifications

• Bachelor's degree or equivalent experience.
• Experience with Seriously and Persistently Mentally Ill (SPMI) population or equivalent experience.
• Fingerprinting and CHC clearance required for Vulnerable Persons Protection.
• Maintain HIPAA privacy compliance and professional client relationships.
• Flexibility to provide shift coverage, including evenings.

Responsibilities

• Maintain professional relationships with clients and comply with HIPAA regulations.
• Conduct initial intake and assessment of tenants and families' needs; ongoing reassessments.
• Prepare and update independent living plans with tenants.
• Identify community resources and make referrals to services.
• Assist tenants in achieving goals and advocating for themselves.
• Conduct group sessions and workshops to develop independent living skills.
• Record progress notes and report on progress in required documentation.
• Follow up with tenants and referral organizations regarding progress and outcomes.
• May schedule appointments and accompany tenants to appointments.
• Input tenant data into automated databases.
