Job summary

The Salvation Army in Mobile, AL is seeking a Full Time Case Manager to support residents in a structured treatment program. This role requires a Bachelor's degree and involves direct client work, documentation, and collaboration with nursing staff.

You will meet with clients, coordinate referrals, monitor progress, and participate in program activities and classes to help individuals regain stability. Regular 40-hour weeks, daily tasks include record keeping and maintaining medication files.

Qualifications

• Bachelor's degree required in Psychology, Social Work, Behavioral Science, or related field.
• Three years progressively responsible experience providing direct case management services or equivalent.
• Equivalent combination of training and experience acceptable.

Responsibilities

• Meets regularly with clients to discuss progress and documents records.
• Outsources to external counseling resources as needed and coordinates with external case management professionals.
• Updates nurse weekly on admissions, hospital changes, and medication changes; assists with refills and appointments.
• Coordinates life-management and behavior-modification classes to support clients.
