Job summary

The Salvation Army USA Central Territory is seeking a Case Manager to provide comprehensive case management, coordinate services, and support clinical staff. You will maintain accurate records, assist with aftercare planning, and participate in reviews and staff development.

Qualifications

• Bachelor's degree in social work or equivalent required.
• Experience in case management preferred.
• Experience with mental health and/or substance use populations preferred.

Responsibilities

• Conducts didactics/educational lectures.
• Advises clients individually or in groups on risk prevention and community resources.
• Establish trusting relationships with patients and families while providing support.
• Assist clients in accessing treatment services and addressing barriers.
• Provide clinical services to screen, assess, and determine appropriate level of care.
• Coordinate access to healthcare, social services, housing and other support.
• Maintain case records in accordance with Harbor Light standards.
• Participates in clinical reviews, staff meetings and training.
