Job summary

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or a related field.
• Three years progressively responsible experience in direct case management and social services.
• Valid State Driver's License.

Responsibilities

• Establishes long-term program goals for clients and counsels on behavior change.
• Outsources to counseling resources and collaborates with external case managers.
• Meets with clients to review progress; maintains records and enforces guidelines.
• Facilitates discussions to motivate clients and provide encouragement.
• Plans and coordinates life management and addiction-related classes; arranges speakers.
• Monitors funding requirements and adjusts case management to meet outcomes.
• Completes discharge plans for clients exiting the program.
• Attends community meetings to educate partners and refer clients.
• Greets and interviews clients to determine eligibility and documents income/expenses.
• Prepares vouchers for food, clothing, furniture for eligible clients.
• Locates temporary housing and coordinates transportation assistance.
• Serves as liaison to other agencies and maintains relationships.
