Job summary

CMJ RECOVERY AZ LLC is seeking a Case Manager to support clinical, non-clinical and medical staff in delivering comprehensive daily programming. You will conduct initial assessments, provide case management services, and assist with aftercare planning, discharge surveys, and documentation.

This in-person role requires HS diploma or GED, valid driver's license with clean record, and strong interpersonal and organizational skills.

Responsibilities

Our employees are the heart of our company, and we look forward to the new addition.Position: Case ManagerWe are looking for a dynamic Case Manager to support clinical, non-clinical, and medical staff in the provision of technical and auxiliary clinical services to ensure comprehensive and effective daily programming and functionality. Specific responsibilities include conducting initial assessments, group counseling, case management services, and aftercare planning.Job DutiesAssists in the development, implementation, and revision of Case Manager treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goalsMeet with patients weekly for Case ManagementCompletes all case management documentation in accordance with regulatory standardsCollect, monitor, and document urinalysis samples and breathalyzer screenings for all CLIA-approved waived testingFacilitates referrals to physicians, dentists, and other healthcare professionals as indicated by the assessed needs of each individual patientProvides transportation in accordance with transportation policy and proceduresConducts Vocational Assessments for clients and ensures follow throughProvides group counseling services in accordance with the established clinical program and completes all documentation accordingly.Collaborates with individual counselors to ensure coordination of appropriate aftercare planning for each clientFacilitates the completion of discharge surveys for clients upon coordination of aftercare planningGathers completed admission, mid-treatment, and discharge surveys with the purpose to aggregate data and compiling a monthly report.Facilitates groups as scheduled by Clinical Supervisor/Clinical DirectorMaintains an active case management caseload providing interventions as needed and within the area of expertise and limits of credentialProvides ASAM-specific reviews as needed and reports findings to the Utilization Review Specialist in a timely and efficient mannerOther duties as assignedRequirements:HS Diploma or GEDA valid driver’s license and a clean driving recordFingerprint Clearance CardStrong interpersonal skillsCase Management experienceKnowledge of all Microsoft office suites including, but not limited to: Word, Excel, PowerPoint, Outlook etc.Versed in the use of Electronic Medical RecordsClinical knowledge pertaining to substance use disordersPreferred Qualifications:Experience in detox, residential, or inpatient medical/behavioral health preferred.Experience performing intake, assessment, treatment planning, and coordination of multi‑disciplinary services.Familiarity with care coordination involving medical providers, therapists, MAT programs, probation/parole, and community resources.Knowledge of withdrawal management, substance use disorders, co‑occurring disorders, and relapse‑prevention support.EHR proficiencyAbility to handle high‑volume communication (email/fax/scanning)Microsoft Office (Word/Excel) proficiencyHigh emotional resilience and calm under crisisStrong organization/time managementAbility to work in fast‑paced detox environmentReliability and follow‑through (critical for legal/medical coordination)Willing to run group with up to 15 clientsJob Type: Full-timeWork Location: In person
