Job summary

American Addiction Centers is seeking a Case Manager to deliver comprehensive patient care, including individual, group, and family counseling. You will conduct initial and ongoing assessments, participate in treatment planning, and work with the UR department to coordinate services for patients.

The role requires a valid driver license, ability to handle a full caseload, and adherence to state standards. This position supports outpatient and inpatient populations and follows 12-hour shift

Qualifications

• High School Diploma or GED required; Bachelor's Degree in Behavioral Sciences preferred.
• Minimum of 2 years experience in a behavioral healthcare field strongly preferred.
• Experience working with substance abuse, addictions, mental health, cognitive behavior treatment and motivational interviewing.
• Knowledge of utilizing electronic health record software and documentation.
• Knowledge of ASAM criteria.

Responsibilities

• Provide substance abuse counseling services in accordance with state laws and guidelines.
• Maintain a full patient caseload and coordinate treatment.
• Provide intervention, assessments, treatment planning, and individual/group counseling.
• Initiate and provide family counseling services.
• Participate in daily treatment team meetings and program staff meetings.
• Ensure documentation meets licensure, accreditation, and policy guidelines.
• Communicate with other departments to ensure best patient care.
• Provide services to outpatient and inpatient adult populations.
• Adhere to HIPAA and 42 CFR Part 2 guidelines.
