COBB OUTPATIENT DETOX LLC is seeking a Clinical Case Manager to support the Clinical Director in coordinating treatment services, discharge planning, and aftercare. You will assess client needs, develop care plans, and advocate for a full range of treatment services within a multidisciplinary team.

The role emphasizes collaboration with healthcare professionals, community resources, and ongoing documentation in EMR to ensure person-centered care and effective aftercare planning in a substance

Qualifications

  • Bachelor’s degree in social work or related field preferred.
  • Proven case management experience in mental health or substance use settings.
  • Strong organizational and communication skills.
  • Ability to collaborate with a multidisciplinary team.
  • Knowledge of community resources and support services.
  • Commitment to diversity, equity and inclusion.
  • Experience in addiction treatment settings preferred.

Responsibilities

  • Conduct comprehensive assessments to identify clients’ needs and develop care plans.
  • Coordinate and advocate for services to meet treatment goals.
  • Collaborate with Clinical Director and healthcare team to ensure continuity of care.
  • Provide support to clients in accessing community resources.
  • Document notes in EMR and communicate discharge and aftercare plans.
  • Facilitate team communication for a cohesive, person-centered approach.
  • Monitor progress toward treatment goals and assist with aftercare planning.
  • Participate in team meetings to discuss client care and plans.
  • Provide crisis intervention and support as needed.
  • Perform other duties as assigned in line with policies.

Position Summary:

The Clinical Case Manager is responsible for assisting the Clinical Director in coordinating treatment services including initial and ongoing coordination of care, advocacy, behavior intervention, aftercare, follow-up, and discharge planning. Case Management will work directly under the Clinical Director to ensure all policies and procedures regarding client care, discharge planning, and case management service needs.

Essential Functions
  • Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
  • Coordinate and advocate for the delivery of a range of services to meet clients' substance abuse treatment goals.
  • Collaborate with Clinical Director, healthcare team and external agencies to ensure continuity of care.
  • Provide support and guidance to clients in accessing community resources and support services.
  • Document all notes into EMR accurately and complete as it pertains to interaction with patients and the communication of their discharge plan and aftercare schedule.
  • Facilitate communication and collaboration among the treatment team to ensure a cohesive and person-centered approach.
  • Monitor and evaluate clients' progress toward treatment goals to assist with aftercare planning.
  • Participate in team meetings to discuss client care and treatment plans.
  • Provide crisis intervention and support as needed.
  • Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.
Qualifications
  • Bachelor’s degree in social work, human services, or a related field, preferred.
  • Proven experience in case management, preferably in a mental health or substance abuse setting
  • Strong organizational and communication skills
  • Ability to collaborate with a multidisciplinary team and external agencies
  • Knowledge of community resources and support services
  • Commitment to promoting a culture of diversity, equity, and inclusion
  • Previous experience in addiction treatment setting or with treatment of co-occurring mental health and substance use disorders preferred
