Job summary

Cahaba Medical Care Foundation seeks a Case Manager in Birmingham, AL to provide comprehensive social services case management for Bibb County Schools and surrounding patients. You will coordinate resources, enrollment in insurance programs, and transportation assistance.

The role involves crisis intervention, referral to community resources, and collaboration with RN/QIO on case management and registry activities.

Qualifications

• Bachelor's Degree in Social Work is required.
• Social Work license or plan to obtain license (must be, at a minimum, registered with the Alabama Board of Social Work Examiners).
• Knowledge of behavioral health and social work concepts.
• Excellent organizational, interpersonal and communication skills.
• Strong computer and internet research skills, including high comfort level with office software products (Google Apps, Microsoft Office, Adobe).
• Ability to work with various levels of internal management and staff as well as outside vendors and clients.

Responsibilities

• Provide social services case management to connect patients with resources through Cahaba Medical Care Foundation programs or partners.
• Administer Sliding Fee discount program with the billing office.
• Assist patients in enrolling in insurance programs (Medicaid, Medicare).
• Help create individualized care plans addressing social determinants of health.
• Coordinate charity care and transportation resources for patients.
