This full-time role focuses on assessing needs, creating care plans, and guiding patients through charity care options and enrollment in appropriate programs, working with clinicians and staff to improve outcomes.

Qualifications

• Bachelor's Degree in Social Work.
• Social Work License or plan to obtain license (registered with Alabama Board).
• Knowledge of behavioral health and social work concepts.
• Excellent organizational, interpersonal and communication skills.
• Proficient with Google Apps, Microsoft Office and Adobe.
• Ability to collaborate with diverse internal and external stakeholders.
• Flexible and adaptable to daily tasks.

Responsibilities

• Administer the Sliding Fee discount program with the billing office.
• Assist uninsured patients to enroll in Medicaid/Medicare.
• Develop individualized care plans addressing social determinants of health.
• Provide case management for charity care programs.
• Coordinate transportation resources for patients.
• Maintain an up-to-date list of community resources.
• Assist with enrollment in Patient Assistance Programs for medications.
