Job summary

Cahaba Medical Care Foundation seeks a dedicated Case Manager to provide social services case management for patients and connect them with resources through CMCF programs or community partners. This full-time role serves the Bibb County Schools.

The Case Manager reports to the Social Services Coordinator and will administer the Sliding Fee program, assist uninsured patients, guide enrollment in Medicaid/Medicare, develop individual care plans, coordinate charity care efforts, and connect

Qualifications

• Bachelor’s degree in social work required.
• Social work license or plan to obtain license (Alabama) required.
• Knowledge of behavioral health and social work concepts.
• Excellent organizational, interpersonal and communication skills.
• Strong computer and internet research skills; proficient with office software.
• Ability to work with internal management, staff, vendors and clients.
• Flexible and willing to help with daily tasks.

Responsibilities

• Administer Sliding Fee discount program with billing coordination.
• Assist front office to identify uninsured patients for sliding fee program.
• Help enroll patients in Medicaid/Medicare and guide enrollment.
• Assist with care planning addressing social determinants of health.
• Provide case management for charity care programs.
• Support providers during clinic hours with social issues.
• Maintain an active list of community resources.
• Assist with obtaining medications via patient assistance programs.
• Manage transportation program for patients and coordinate resources.
• Provide crisis intervention services.
• Refer clients to community resources and advocate on patient’s behalf.
• Complete training to become a Certified Application Counselor.
• Collaborate with RN/QIO on Case Management/Registry.
• Assist with fundraising, community development or outreach.
• Other duties as assigned.
