Qualifications

• Bachelor's Degree in Social Work required.
• Social Work License or plan to obtain license (AL Board).
• Knowledge of behavioral health and social work concepts.
• Excellent organizational, interpersonal and communication skills.
• Strong computer and internet research skills with office software.

Responsibilities

• Administration of the Sliding Fee discount program with the billing office.
• Assist uninsured patients and others in need of Sliding Fee Discount program.
• Identify patients eligible for Medicaid/Medicare and assist enrollment.
• Develop individualized care plans addressing social determinants of health.
• Provide case management for charity care programs (e.g., Bibb Charity Care).
• Support providers with social issues affecting patient well-being.
• Maintain an active list of community resources for patients.
• Assist with medication access through Patient Assistance Programs.
• Manage transportation program for patients and explore options.
• Provide crisis intervention services to patients in need.
• Refer clients to community resources and advocate with agencies.
• Train to become Certified Application Counselor with Marketplace and Medicaid.
• Collaborate with RN/QIO on Case Management/Registry Management.
• Assist with fundraising, community development or outreach as needed.
