Job summary

Cahaba Medical Care is seeking a Case Manager to provide comprehensive social services case management for patients and to connect them with resources through CMCF programs and community partners. The role focuses on helping patients access charity care and insurance enrollment, navigate transportation and medication access, and support care plans addressing social determinants of health.

Qualifications

• Bachelor's degree in Social Work required.
• Social Work license or plan to obtain license (registered with Alabama Board of Social Work Examiners) required.
• Knowledge of behavioral health and social work concepts.
• Excellent organizational, interpersonal and communication skills.
• Strong computer skills with Microsoft Office, Google Apps, and Adobe.
• Ability to collaborate with internal management and external vendors/clients.
• High flexibility and willingness to handle daily tasks.

Responsibilities

• Administer Sliding Fee discount program with billing office, including uninsured/sliding scale fee plans.
• Identify uninsured patients and others needing Sliding Fee Discount assistance.
• Assist in enrollment of patients in Medicaid/Medicare and walk through enrollment process.
• Assist with care planning to address social determinants of health for chronic disease management.
• Provide case management for charity care and access to specialty care.
• Support providers during clinic hours with social issues affecting patient well‑being.
• Maintain an active local community resources list for patient use.
• Assist patients with obtaining medications through Patient Assistance Programs and low-cost options.
• Manage transportation program for appointments and refer to additional resources as needed.
• Provide crisis intervention services and refer clients to community resources.
• Complete training to become a Certified Application Counselor with Marketplace and Medicaid.
• Collaborate with RN/QIO on Case Management/Registry Management for patients.
• Support fundraising, community development or outreach efforts.
