Job summary

Volunteers of America–Greater New York (VOA–Greater New York) seeks a Case Manager to support families experiencing homelessness at Newton Hotel Family Shelter in Manhattan, NY. You will assess client needs, develop service plans, coordinate referrals, and provide advocacy and counseling to help clients access needed benefits.

Qualifications

• Associates Degree or 60 college credits in Human Services.
• High School Diploma or GED with 2 years shelter experience.
• Strong verbal and written communication skills; basic computer skills.

Responsibilities

• Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings and goals; update as required.
• Meet regularly with the client to assist in obtaining needed services/benefits and provide supportive counseling to strengthen decision-making.
• Document case management activities and collect service data for evaluation and quality assurance.
