Job summary

Volunteers of America–Greater New York (VOA–Greater New York) is seeking a Case Manager to work with clients experiencing domestic violence, assessing needs, planning actions, linking to services and monitoring progress in our NYC-area programs.

The successful candidate will have an associate degree or 60 college credits in Human Services or a high school diploma with shelter experience, plus strong verbal and written communication skills and basic computer abilities.

Qualifications

• Associate's degree or 60 college credits in Human Services, or HS Diploma/GED with 2 years shelter experience.
• Strong verbal and written communication abilities and basic computer skills.
• Candidates with related domestic violence experience preferred.

Responsibilities

• Manage the client intake and assessment process.
• Develop an individualized service plan with the client and update as required.
• Meet regularly with the client to assist in obtaining needed services/benefits and provide supportive counseling.
• Document case management activities per agency and funder guidelines and participate in data collection and QA.

Position: Case Manager

The Case Manager works with clients experiencing domestic violence to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress.

Minimum Qualifications:

• Associate's degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 2 years of relevant shelter experience.
• The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.

Candidates with related experience with domestic violence preferred.

Case Manager Principal Responsibilities:

• Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
• Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
• Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
