Volunteers of America–Greater New York (VOA–Greater New York) seeks a Case Manager to assess client needs, plan services, and link clients to supportive resources at the Wards Island location. The role includes counseling, referrals, and advocacy to achieve outcomes.

The position emphasizes strong communication, documentation, and collaboration with shelter staff to support clients in securing services and benefits.

Qualifikationen

• Associates Degree or 60 college credits in Human Services or HS diploma/GED with 2 years shelter experience.
• Demonstrated verbal and written communication skills.
• Basic computer skills.

• Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings and goals.
• Meet regularly with the client to assist in obtaining needed services/benefits and provide supportive counseling.
• Document case management activities per agency and funder guidelines and participate in evaluation activities.

We are currently seeking qualified candidates for the following position:

Position: Case Manager

Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. The case manager must assure that clients receive appropriate, competent and professional information and advice, referrals and advocacy services to achieve successful outcomes. May include supervision of direct care staff.

The Schwartz Men’s Shelter is a shelter for men operating 24 hours a day and seven days a week. Clients are referred to the program by the New York City Department of Homeless Services. To learn more about the program, copy and paste this link into a browser https://www.voa-gny.org/housing

Minimum Qualifications:

• Associates Degree or 60 college credits courses in Human Services field or High School Diploma or GED with 2 years of relevant shelter experience.
• The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.

Case Manager Principal Responsibilities:

• Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
• Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
• Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
