Job summary

Family Service League is seeking a full-time Case Manager for our large Family Shelter. The role conducts client assessments, develops Independent Living Plans, and links families with community resources to promote self-sufficiency through advocacy, counseling, and referrals.

Qualifications

• High School diploma required; bachelor’s degree in human services or related field or a master’s degree preferred.
• Minimum of 2 years’ experience in human services or related field.
• Knowledge of homeless population and DSS policies and regulations.
• Excellent interpersonal and verbal and written communication skills.
• Strong organizational and time management skills.
• Proficient computer skills including Microsoft Office; HMIS a plus.
• Spanish language fluency preferred.
• Valid and clean New York State Driver's license required.

Responsibilities

• Complete new client intakes and explain shelter rules and regulations to clients.
• Meet with clients a minimum of two times per week for face-to-face meetings.
• Develop Independent Living Plan for assigned families.
• Complete Independent Living Plan bi-weekly.
• Complete Mental and/or Physical Impairment form with Independent Living Plan Violations.
• Complete SPA applications for eligible clients.
• Work collaboratively with Housing Specialists in finding permanent housing options for clients.
• Conduct client safety inspections twice a week.
• Address safety issues in unit and remove dangerous items.
• Address unit cleanliness and client hygiene and assist daily living skills.
• Collaborate with the Office Manager on shelter obligations.
• Create a budget with clients and identify needs and obligations.
• Review and submit FSL Client Incident Reports to the Compliance Supervisor.
• Maintain resource database of social service organizations.
• Provide ongoing support and counseling for clients.
• Foster inter-agency collaboration for long-term stability.
• Maintain HMIS and client files documentation.
• Complete monthly reports and submit to DSS Housing Liaison.
• Attend Case Management team meetings and trainings.
• Oversee shelter rules compliance; ensure confidentiality and HIPAA.
• Provide assessment, crisis intervention, and services to children, youth, and families.
• Advocate for client needs and provide transportation as needed.
• Collaborate with treatment/service coordination teams; work with security on incidents.
• Read and respond to daily emails and manuals.
• Essential position requiring presence during inclement weather and emergencies.
• All other duties as assigned.
