Job summary

Family Service League Inc. in Smithtown, NY is seeking a part-time Case Manager for a homeless shelter.

The role involves completing new client intakes, explaining shelter rules, and developing Independent Living Plans with clients. You will meet clients at least twice weekly, coordinate with Housing Specialists to find housing, provide advocacy and crisis intervention, maintain HMIS documentation, and contribute to program outcomes.

Qualifications

• Bachelor’s degree in Human Services required.
• Master’s degree in Social Work preferred.
• Minimum of 1 year of experience in human services or related field preferred.
• Knowledge of the Suffolk County Department of Social Services preferred.
• Excellent interpersonal, verbal, and written communication skills required.
• Valid NYS Driver's License required.

Responsibilities

• Complete new client intakes and explain shelter rules to clients.
• Meet with clients at least twice weekly for face-to-face meetings.
• Develop Independent Living Plans for clients.
• Collaborate with Housing Specialists to find permanent housing options.
• Maintain HMIS documentation and client files.
• Provide crisis intervention and ongoing client support.
