Family Service League Inc. in Brentwood, NY, seeks a full-time Case Manager for its large Family Shelter.

You will conduct client assessments, develop Independent Living Plans, link families to community services, and support client self-sufficiency through advocacy, counseling, and resource referrals. You will collaborate with the Housing Specialist, perform crisis intervention, conduct safety checks, and help clients create budgets while maintaining HIPAA and shelter rules.

Qualifications

• Minimum 2 years' experience in human services.
• Knowledge of homeless populations and DSS policies.
• Excellent interpersonal and verbal and written communication skills.
• Valid New York State Driver's license required.

Responsabilités

• Complete new client intakes and explain shelter rules and regulations to clients.
• Meet with clients a minimum of two times per week for face-to-face meetings.
• Develop Independent Living Plan for assigned families.
• Collaborate with Housing Specialists in finding permanent housing options for clients.
• Conduct client safety inspections twice a week.
• Address safety and cleanliness issues in units and assist client daily living skills.
