Job summary

Family Service League is seeking a part-time Case Manager for a homeless shelter in Smithtown, NY. The Case Manager will conduct client assessments, developing Independent Living Plans, linking clients with community-based service resources.

Qualifications

• Bachelor's degree in Human Services required; MSW preferred.
• Minimum 1 year of experience in human services or related field preferred.
• Knowledge of Suffolk County Department of Social Services preferred.
• Knowledge of human services and working with at-risk populations preferred.
• Proficient computer skills; HMIS, Google Apps, and project management software preferred.
• Valid NYS driver's license required.

Responsibilities

• Complete new client intakes and explain shelter rules.
• Meet with clients at least twice weekly for face-to-face meetings.
• Develop Independent Living Plans for clients.
• Complete SPA applications for eligible clients.
• Collaborate with Housing Specialists to find permanent housing.
• Conduct twice weekly client safety inspections.
• Maintain HMIS documentation and client files.
• Provide crisis intervention and counseling to clients.
• Prepare monthly reports and submit to the DSS Housing Liaison.
• Attend staff meetings and trainings; enforce shelter rules.
