Job summary

Ava Health in Grand Junction, Colorado seeks a Case Manager to join its interdisciplinary clinical team. You will address social determinants of health, coordinate community resources, and support discharge and aftercare planning to empower clients toward independence.

You will maintain caseloads, complete assessments, document care plans, and collaborate with clinicians, residential staff, and community partners while upholding confidentiality and person-centered care.

Qualifications

• Bachelor's degree preferred but not required; relevant case management or behavioral health experience may count.
• Experience in behavioral health, human services, case management, or related setting preferred.
• Strong communication, organization, and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
• Comfortable working with individuals with mental health and/or substance use disorders.

Responsibilities

• Maintain an assigned case management caseload.
• Complete case management and social determinants of health assessments.
• Meet regularly with clients to set goals and address barriers to treatment.
• Assist clients with housing, transportation, benefits, employment, and other resources.
• Coordinate referrals and appointments with outside providers.
• Collaborate with clinicians, nursing, and residential staff.
• Coordinate discharge and aftercare planning and follow-up.
• Encourage client independence and self-advocacy.
• Complete case management notes and required documentation accurately.
• Maintain professional boundaries and confidentiality.
• Identify safety concerns and escalate promptly.
