Qualifications

• Excellent verbal and written communication with professional phone etiquette.
• Minimum of 1 year Case Management experience preferred

Responsibilities

• Track and record patient case information, progress, and evaluate recovery.
• Meet with patients and families to understand needs and recommend care with treatment plans.
