Health Solutions West in Grand Junction, CO, seeks a dedicated Case Manager to support a diverse patient population within our behavioral health program. The role emphasizes discharge planning, aftercare, and coordinating services to promote lasting recovery for children, adults, couples, and groups.

You will collaborate with a clinical team, document using Windows-based tools, and travel between Health Solutions West sites as needed.

Qualifications

• Bachelor’s degree required; preferably in human services or related field.
• One year experience in a healthcare field acceptable if degree is non-related.
• Two years human service related experience preferred.
• Current valid Colorado Driver’s License required.
• Certified Addiction Counselor II or III strongly preferred.

Responsabilités

• Utilizes a variety of treatment approaches to work with a diverse population.
• Provides therapeutic and case management services, specifically discharge planning and aftercare of patient hospitalization.
