Job summary

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• Minimum two years of case management experience.
• Experience coordinating with clinical teams and external resources.

Responsibilities

• Receives referrals and assesses client needs for case management and treatment.
• Links clients to community resources and provides case management monitoring.
• Advocates for client access to resources and coordinates with primary clinicians.
• Documents treatment progress and participates in supervision and planning meetings.
• Maintains professional demeanor and manages intake processes with accuracy.
