VOA Northern Rockies in Wyoming seeks a dedicated Case Manager to support assigned clients, assess needs, and link them to appropriate community services in coordination with the treatment team. You will help navigate resources and monitor progress within a residential setting.

Responsibilities include advocacy for access to services, liaison work with hospitals and partners, timely documentation, and participation in supervision and planning discussions to support treatment goals.

Formación

• Bachelor's degree in psychology, social work, or related field.
• Minimum two years of case management experience.
• Experience coordinating with clinical teams and external resources.

Responsabilidades

• Receives referrals and assesses client needs for case management and treatment.
• Links clients to community resources and provides case management monitoring.
• Advocates for client access to resources and coordinates with primary clinicians.
• Documents treatment progress and participates in supervision and planning meetings.
• Maintains professional demeanor and manages intake processes with accuracy.

Conocimientos

Educación

Bachelor's Degree in Psychology, Social Work, or related field

Two or more years of Case Management experience
