Job summary

You will advocate for access to resources, coordinate to ensure seamless care, and document outcomes while maintaining confidentiality and compliance with HIPAA and agency policies. The role involves community-based and office-based work within a supportive team.

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Counseling, Sociology, Behavioral Health, or related field required.
• 1 year of case management experience preferred.
• Minimum two years of experience supporting individuals in behavioral health, addiction recovery, mental health, or related settings preferred.
• Valid driver's license with acceptable driving record required.
• Ability to work effectively in both office and community-based settings.

Responsibilities

• Assesses participant needs, strengths, barriers, and goals to inform planning.
• Develops, implements, and monitors individualized service plans.
• Advocates for access to services and community resources to support recovery.
• Coordinates services among providers, families, and supports networks.
• Documents progress and ensures HIPAA-compliant records.
