Job summary

Encompass Health is seeking a Case Manager in Colorado Springs to coordinate patient care from admission to post-discharge, while working with an interdisciplinary team to develop and monitor treatment plans. You will ensure timely discharge planning and resource coordination to support patient outcomes.

The role requires independent assessment capabilities, RN licensure (BSN preferred), and CCM/ACM certification within two years.

Qualifications

• Independent assessment ability within scope of practice.
• CCM or ACM certification required or obtainable within two years of entry.
• Bachelor’s degree and/or nursing licensure with preferred BSN.
• Rehabilitation experience is preferred.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Collaborate with interdisciplinary team to manage treatment plans.
• Assist with discharge planning and post-discharge resources.
• Monitor and document case management activities and outcomes.
• Ensure compliance with regulatory requirements and company policies.
