Job summary

Volunteers of America Northern Rockies seeks a Case Manager to support assigned clients and connect them with community resources. The role includes initial and ongoing assessments, referrals, and collaboration with therapists and the treatment team.

The position involves providing psychiatric rehabilitation services as part of a treatment plan, monitoring client progress, and advocating for access to services while maintaining thorough documentation.

Qualifications

• Bachelor's Degree in Psychology, Social Work, or related field.
• Two or more years of Case Management experience.

Responsibilities

• Receives referrals for Case Management services/treatment from therapists and assesses client needs.
• Provides case management monitoring of the Client's condition and progress and communicates with the primary clinician.
• Advocates on behalf of the Client to ensure access to needed resources and protect rights.
• Provides liaison services to state/local hospitals and community partners and attends meetings as needed.
• Maintains documentation reflecting the course of treatment and manages intake elements and releases.
