Job summary

Johns Hopkins Medicine is seeking a Home Care Coordinator Discharge Planner to coordinate pediatric home care services as patients transition from hospital to home. This role acts as a discharge planning resource without hands-on clinical care, based at Johns Hopkins Care at Home.

The ideal candidate holds a BSN, RN licensure, active CPR, and 2+ years of clinical nursing experience with at least 1 year in case management; home care experience preferred.

The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for pediatric patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.

Work Location: JHH

Weekly Working Hours: Days, 40 hours per week

Qualifications:

• Bachelor of Science in Nursing (BSN) required
• Licensure in clinical discipline (RN)
• Active CPR
• Two (2) years experience in clinical nursing required , mininum 1 year of previous case management required, previous home care or home care discharge planning experience preferred
• Knowledge of Medicare Conditions of Participation preferred
• Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.

Salary Range: Minimum 47.96/hour - Maximum 74.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
