Job summary

TASC (Treatment Alternatives for Stronger Communities) is hiring a full-time Case Manager in the Chicago area. You will coordinate addiction intervention, AODA counseling, and recovery support for clients and families, linking behavior health resources and public benefits to service plans.

You’ll work with diverse caseloads, including high-risk clients, to help achieve established goals and progress milestones in a collaborative, community-based setting.

Qualifications

• HS diploma or GED required.
• Valid driver's license and access to a personal vehicle.
• CADC certification within two years of employment may be required.
• Must pass an IDOC background check.
• Proficient in Microsoft Office.
• Experience in group facilitation.
• Knowledge of ASAM criteria, DSM‑V related to substance use, or ability to learn.
• Comfort working in a correctional setting with offenders.
• Flexible in thinking and able to lead group discussions.
• Strong verbal, written, and interpersonal communication.
• Good time management with ability to meet deadlines.

Responsibilities

• Assist individuals in achieving service plan goals via coordination of services.
• Assess and monitor progress relative to service plan and parole requirements.
• Complete intake packets for program enrollment.
• Provide referrals and crisis intervention for identified needs.
• Interview individuals and review records with professionals for suitability.
• Collaborate with partners and participate in staff meetings and supervision.
• Identify and refer individuals with behavioral health needs.
• Complete and maintain all required program documentation and reports.
• Ensure confidential handling of information and reporting.
