Job summary

Providence of Maryland, Inc. is seeking a Case Manager for the Personal Supports program. You will manage Person-Centered Plans, coordinate with families and staff, and ensure services align with goals, regulations, and Providence standards.

You will monitor documentation, provide coaching, and promote consistent, person-centered service delivery across the Personal Supports program. Strong communication and organizational skills are essential.

Qualifications

• Bachelor’s degree or 4 years of similar experience.
• Previous case management experience preferred.
• Knowledge of Microsoft Office Suite and agency/state systems; Therap, PCIS2.

Responsibilities

• Develop, implement, and monitor Person-Centered Plans (PCPs) with the person and support network.
• Serve as the primary case management contact for individuals and communicate with families and interdisciplinary teams.
• Conduct routine check-ins with people supported, families, and caregivers to assess satisfaction and accountability.
• Educate Providence staff on PCPs and provide guidance on implementation of supports.
• Monitor documentation for accuracy and regulatory compliance in LTSS systems.
• Coach staff and address performance or service concerns with leadership guidance.
