Job summary

Providence of Maryland Inc. seeks a dedicated Case Manager for the Personal Supports program. You will partner with Direct Support Professionals, families, and interdisciplinary teams to ensure goal-aligned, person-centered service delivery. The role emphasizes guidance, coaching, and regulatory compliance across documentation standards.

You will monitor PCP development, communication with families, and staff performance while upholding Providence standards and quality of care.

Qualifications

• Bachelor’s degree or 4 years of similar experience.
• Previous case management experience preferred.
• Knowledge of Microsoft Office Suite (Word, Excel, Outlook) and Therap/PCIS2, state systems.
• Physical requirements include lifting up to 40 lbs.

Responsibilities

• Develop, implement, and monitor PCPs with person and support network.
• Serve as primary case management contact for individuals.
• Conduct routine check-ins to assess satisfaction and ensure continuity of care.
• Educate Providence staff on PCPs and implementation.
• Monitor service implementation and ensure supports align with Person-Centered Plans.
• Identify performance or documentation concerns and coach staff.
• Ensure staff understand Providence policies and regulatory requirements.
• Collaborate with supervisors to promote consistency and quality.
• Monitor documentation in iCare Manager, LTSS, and other systems to ensure accuracy.
• Audit documentation and provide coaching to improve quality.
