Position Summary
Thecase manager, person-centered services positionwillprovide case coordination and advocacy forindividuals with disabilities supported byOBI who are actively participating in current services or who are seeking to transition into services. This position will help individuals and families navigate the "pathway" toservices andbecomes a critical member of an individual’s Person-Centered Planning Team. This coordinator, person-centered services willhave extensive knowledge of OBI's funding and referral sources (i.e., DDA, DORS, CCS's, schools, etc.), regulations and will establish and maintain excellent professional relationships with all stakeholders. We are offering a salary range for this position of $50K - $58K.
Essential Functions & Responsibilities
- Provide case coordination services tothe participantsin OBI programingbyensuringmeaningful programing and the highest quality of care and support through theperson-centeredplanning (PCP) process.
- Work collaboratively with Coordinators of Community Services (CCSs) to facilitate timely Person-CenteredPlanning(PCP)meetings.
- Work with each participant and their team to discuss and establish person-centered goals to be included in the Service Implementation Plan (SIP).
- Maintain close communication with OBI direct support professionals to assist with program planning and monitor the implementation and progress toward PCP goals.
- Develop and submit all required documentation to request and secure appropriate DDA funding for each individual participant.
- Monitorparticipant documentation within OBI's online database platform for accuracy, quality, and to remainup-to-dateon each participant's progress.
- Involve OBI's Workforce Development team whenever a participant is interested in employment services.
- Involve OBI's Manager, Personal Supports whenever a participant is interested in Personal Supports services from OBI.
- Complete the attendance process as determined by OBI.
- Review and approve required documentation on time, including but not limited to OBI's online database platformservice logs.
- Review PCP's on LTSS for accuracy and provide feedback to CCS and/or Manager of Person-Centered Services regarding any necessary changes that need to be made before services can be accepted.
- Review and authorize services for participants in LTSS.
- Assure Behavior Plan compliance by assisting the Behavior Specialist.
- When requesting/advocating for individual services, provide DDA and/or the CCS with relevant and substantial back-up documentation and data to justify the request.
- Train staff appropriately for eachparticipanton caseload that requires specific training and maintain required documentation thatthe training occurred.
- Example: Specialized protocols for feeding or toileting,documented in the PCP.
- Maintain accurate and complete records for each participant as instructed inthe client management system.
- Follow allDDA and OBIregulations when investigating, monitoring, and reporting incidents.
- Assist to assure quality services and agency regulatory compliance.
- Provide back-up transportation as needed for participants.
- Provide Direct Care as needed for participants.
- Communicate with families/caregivers, andCCSson a regular basis, being sure to communicate any changes or important information with the entire team.
- Attend regulardepartmentalmeetings.
- Intervene in crisis situations to assistparticipantsin resolving time-sensitive issues; as well as ensuring the appropriate services are received.
- Establishrelationships with other appropriate community nonprofits and service providers, to collaborate and share resources.
- Maintain up-to-date knowledge of DDA funding, service definition and regulations.
- Maintain compliance with all OBI, DDA, OHCQ, and DORS policies and regulations at all times.
- Maintain the participant files on the server.
- Submit all OBI forms to the CCS as part of the PCP.
Non-Essential Duties
- Attend external meetings, conferences and trainings asrequested by supervisor.

Qualifications
- Minimum of1year of successful experience working with individuals who have disabilities, providing case coordination/case management, or serving in a quality assurance or compliance capacity.
- High school diploma or equivalent required; Bachelor's degree in a related field preferred.
- Excellent written and oral communication skills.
- Highly organized and demonstrated ability to work well independently and as part of a team
- Experience in using a client management system. iCare Manager preferred.
