Job summary

Community Solutions in Barstow, CA seeks a dedicated Case Manager for the MAP Office to engage justice-involved individuals with rehabilitative programming. You will perform intake assessments, counseling, and case management to support client progress at the Barstow MAP site.

Qualifications

• Bachelor’s degree in a social science field and one year experience in case management or similar field.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Conduct client orientation, intake data, needs assessments and make recommendations.
• Develop program goals, monitor stipulations, job readiness, referrals, budgeting.
• Develop and implement discharge plans for clients.
• Schedule and document individual counseling sessions addressing client issues.
• Facilitate group meetings on substance abuse and pre-release issues.
• Maintain client files and ensure confidentiality and log all activities.
• Provide Aftercare services and monitor program performance.
