Job summary

Community Solutions, Inc. (CSI) is seeking a dedicated Case Manager in Barstow, CA, to coordinate intake assessments, counseling, and discharge planning within the MAP Office.

The role focuses on rehabilitative programming for justice-involved individuals and developing long-term, self-sustaining skills through vocational training and personal development services. The position entails handling client files, group and individual counseling, and aftercare planning, with a Monday–Friday schedule

Qualifications

• Bachelor’s degree in a social science field; one year of case management or related experience, substitution allowed on a year-for-year basis.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Conduct client orientation and intake data collection; assess needs and advise supervisor.
• Develop and document program goals with clients; monitor treatment and referrals.
• Develop discharge plans and coordinate aftercare services.
• Schedule and document individual counseling sessions addressing client needs.
• Maintain confidential client files and complete progress notes and summaries.
• Provide group counseling and assist in staff supervision as needed.
• Complete documentation and log entries for all shift activities.
