Breaking Ground is seeking a Case Manager, Connect to Care, to provide outreach and case management for people experiencing homelessness. You will conduct psychosocial evaluations, help access benefits, and support transitions to permanent housing.
The role requires a Bachelor’s degree or equivalent experience, at least two years in homelessness services, and comfort with harm reduction and motivational interviewing techniques. A driver’s license is preferred.

Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Reporting to the Social Services Supervisor, the Case Manager, Connect to Care provides outreach and case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.
This position has a 35-hour work week.
