Breaking Ground seeks a Case Manager, Connect to Care PFNYC to provide outreach and case management for people experiencing homelessness. You will assist clients in transitioning to permanent housing, conduct psychosocial evaluations, and connect them with psychiatric and medical services.

Reporting to the Social Services Supervisor, theCase Manager, Connect to Care PFNYC provides outreach and case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.
This position has a 35-hour work week.
