Job summary

Copa Health is seeking a Case Manager Lead to help develop integrated assessments and ISP, coordinating with healthcare and behavioral health providers to implement services and track progress. You will document member progress and support recovery goals.

Qualifications

• Bachelor’s degree in behavioral health related field and 2 years’ of full-time behavioral health work experience.
• Bachelor’s degree in non-behavioral health related field and at least 4 years’ of full-time behavioral health work experience.
• High school diploma or high school equivalency diploma and 6 years’ of full-time behavioral health work experience
• Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
• AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.

Responsibilities

• Provides mentorship and training to Case Manager new hires and trainees, i.e., EMR-Next Gen, Tele-Health, Shadowing Expert, etc.
• Works collaboratively with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the Integrated Service Plan are implemented.
• Develops and maintains and manages the Integrated Service Plan (ISP) within required timeframes.
• Adjusts the ISP and service delivery as needed based on progress/lack of progress in the specified health, wellness and behavioral health goals.
• Provides assistance in maintaining, monitoring and modifying covered behavioral health and healthcare services and community resource needs.
• Maintains the person’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
