Qualifications

• Master's degree and PA licensure (LSW or LAC) required.
• Experience as peer guide, shift-based lead, or workflow coordination.
• Knowledge of federal/state behavioral health/hospital operations.
• Must pass background checks and be vaccinated.
• Credentialing requirements (NPI, Medallion, Medicare) within required time.
• Ability to perform and certify Safe Clinch and Therapeutic Options restraints.

Responsibilities

• Manage daily patient cases across walk-in, Observation, and CSU units with individualized stabilization or treatment plans.
• Complete, review, and obtain signatures for treatment plans with patients and involved parties.
• Gather collateral information from patients, family, and others to support assessments and discharge planning.
• Coordinate patient care with interdisciplinary team to ensure integrated services.
• Identify service gaps and facilitate referrals and discharge planning for continuity of care.
• Monitor unit census and community placement options to support timely flow.
• Maintain accurate clinical documentation in the medical record and ensure compliance.
• Provide shift-based workflow guidance and regular reports to leadership.
